Please fill out the following form and pay online through PayPal with a credit card (no PayPal account required) to register for the event online.
Favor de llenar el siguiente formulario y pagar por Internet por PayPal con una tarjeta de crédito (no es necesario tener una cuenta de PayPal) para registrar para el evento en el Internet.
Early bird registration deadline is April 23rd 2011. A $10 late fee will be added to each runners application fee afterwards.

I personally assume all risks incident to my child's participation in the Awesome 3000, presented by the Salem-Keizer
Education Foundation, including, but not limited to: use of facilities; use of streets, and all hazards from participation
in this event. I hereby release for myself, my child and his/her heirs, executors and administrators, all sponsors and
co-sponsors of the event, their officers, agents and volunteers, from any and all claims, demands, action or causes of
action incident to my child's participation in this event. I grant permission to all of the foregoing to use any photographs,
motion pictures, video recordings, or any other record of this event for any purpose,
I understand that the entry fee is nonrefundable in any circumstance and that my entitlement to run materials and subsequent
permission to participate expires at 8:00pm the Friday before the event.
Yo asumo los riesgos que implica la participación de mi niño en Awesome 3000, patrocinado por la Fundación Educativa de Salem-Keizer, incluyendo pero sin limitar: el uso de las instalaciones, calles y los peligros que acarrea la participación en esta actividad. Por este medio descargo de responsabilidad para mí mismo, mi estudiante y sus herederos, ejecutores y admininstradores, a todos los patrocinadores y copatrocinadores del evento, sus oficiales, agentes y voluntarios, de cualquier reclamación, demanda, acción o causa de acción relacionadas con la participación de mi estudiante en este evento. Yo concedo permiso a los mencionados anteriormente a usar cualquier fotografias, películas, cintas de video o cualquier otra grabación del evento para cualquier propósito.
Entiendo que la tarifa de entrada no es reembolsable bajo ninguna circunstancia y que mi derecho a usar materiales y subsecuente permiso para participar expira a las 8 p.m. el viernes antes del evento.
